I expect several ladies here know more about this than me but basically they are manual ball winders.
The end of your yarn goes round the groove at the end and then you hold the handle and wind your yarn into a ball in a figure-of-eight motion at the tapered end. The end off the nostepinne is gently tapered to make it easy to slide your ball of yarn off, causing the inside end to dangle. You can then knit or whatever from the inside of your yarn out, reducing the risk of tangles etc.
If you google 'nostepinne' there is probably a more explicit explaination there.
